Just About Ready to Apply for Accreditation

signatureLHDs that are ready to apply for accreditation will find tips from peers familiar with accreditation as well as links to Public Health Accreditation Board (PHAB) materials and other resources to help with transitioning from preparing for accreditation to application.

  • PHAB Materials: These documents are currently under development. Check the PHAB website for updates on the availability of accreditation materials.
  • Example documentation: NACCHO has collected examples of documentation that LHDs are using to meet PHAB standards and measures. Visit the example documentation for accreditation page to view the examples.
  • Case Examples: Several stories from LHDs preparing for accreditation (including  North Carolina, Tulsa City-County Health Department, OK, and Polk County Health Department,WI) can be found on the Case Examples webpage.
  • Organizing Your Documentation:  When collecting documentation to show conformity to accreditation standards and measures it is useful to create an internal system to organize the documentation and streamline the process. PHAB has created an editable documentation organization spreadsheet that agencies can personalize or adjust to their needs.
