Million Hearts

As a member of the CDC Million Hearts Collaborative, NACCHO invites local health departments (LHDs) to be a part of Million Hearts - a national initiative with the ambitious goal of preventing one million heart attacks and strokes by 2017. NACCHO will provide LHDs with the information and resources to educate the public on the importance of preventing heart attack and stroke in the United States.

In the Spotlight

Million Hearts® and the Centers for Disease Control and Prevention are pleased to announce the launch of a new campaign aimed at preventing heart attacks and strokes. Cardiovascular disease is the leading cause of death in the United States, and African-American men are more likely to die from heart disease than non-Hispanic white men. To help address this health disparity, the CDC Foundation is partnering with Pfizer and Quest Diagnostics to launch the Million Hearts®"Healthy is Strong" initiative. "Healthy is Strong" will raise awareness about lifestyle changes and medical condition management among African-American men for prevention of cardiovascular disease, including talking to a health care provider about the ABCS of heart health. The campaign promotes patient engagement with the healthcare system and includes a suite of culturally-tailored educational materials for consumers and health care providers.  More »

Archived Webinar - Creating Community-Clinical Linkages to Address & Prevent Chronic Disease

Join NACCHO and Summit County Public Health (Summit County, Ohio) on the upcoming webinar highlighting robust referral networks linking primary care providers with public health and community resources that can be an effective model for improving health outcomes and promoting health equity for all populations. During this webinar, Summit County Public Health will describe their experiences working with local and state partners to build bi-directional referral networks between public health and primary care physicians.

Archived Webinar - Million Hearts and Sodium: Making Sense of the Science and Translation to Practice

NACCHO, the American Heart Association (AHA) and Association of State and Territorial Health Officials (ASTHO) hosted a webinar focusing on the public health rationale for reducing population sodium intake and recent research that has called sodium recommendations into question. The aim was to help health professionals make sense of the sodium science, clarify confusion about guidelines regarding salt and health, and provide state and local examples of successful implementation of sodium reduction guidelines.

Million Hearts Lessons from the Field: Improving Quality and Accuracy in Blood Pressure Monitoring

This webinar provided an overview of evidenced-based hypertension protocols developed by and used in four local health departments in the United States as one of the key strategies for impacting cardiovascular disease prevention. APHN provides consultation on the role and contribution of the PHN, evidenced based resources as well as case studies of public health nurses impacting cardiovascular disease.

Learning Objectives:

  • Identify factors involved in proper blood pressure measurement in screening for hypertension.
  • State the roles of community/public health nurses in blood pressure measurement.
  • List evidenced-based protocols develop and being used in practice.
  • Increase awareness and understanding of local efforts to implement the Million Hearts initiative in cardiovascular disease prevention.

Success Story - Practice-Based Successes in Sodium Reduction in Local Communities

Read a success story from Shawnee County Health Agency (SCHA), which aimed to increase access to lower sodium food options, reduce sodium intake, and continue to build effective evidence- and population-based strategies to reduce sodium consumption at the community level. SCHA worked with different stakeholders to form the Sodium Reduction Leadership Team to promote salt reduction in foods sold in local gas stations and the zoo.

New Resource: Million Hearts Local Engagement Guide

MHGuideThe Million Hearts® Local Engagement Guide was developed for local health departments that work with a wide range of community partners to create conditions and policies that help people make healthy choices, such as avoiding tobacco use, becoming more physically active, and eating healthier foods. All these proactive measures contribute to the prevention of chronic diseases such as obesity, diabetes, and heart disease. This guide will assist local health departments when engaging government partners, health care providers, the public, and other stakeholder groups at the regional, state and local level to advance Million Hearts.
