Action Communities for Health, Innovation, and EnVironmental changE (ACHIEVE) was a collaboration of the Centers for Disease Control and Prevention (CDC), NACCHO, and other selected national organizations, addressed obesity and other chronic disease risk factors communities creating and enhancing policy, systems, and environmental improvement strategies such as increasing access to healthy foods and encouraging physical activity in local communities. NACCHO funded 33 communities and facilitated development of a community action plan, linked communities to resources, provided technical assistance, supported the evaluation process, and maintained the ACHIEVE website. 

The communities addressed strategies that can prevent or help manage the health risk factors for heart disease, stroke, diabetes, cancer, obesity, and arthritis. Specific activities were directed toward reducing tobacco use and exposure, promoting physical activity and healthy eating, and improving access to consistent high-quality preventive health services.

In the May/June 2013 issue of the Journal of Public Health Management and Practice, the article "Multisector Coalitions Build Healthier Communities Through ACHIEVE Initiative" features new research on the success of NACCHO-funded ACHIEVE communities in addressing chronic disease.
