Essential Service Designation Example - Rockland County


This six page PDF documents was created in 2006 as evidence for the Measure of Essential Service Designation (ESD) for Project Public Health Ready (PPHR) by the Rockland County Department of Health.  

The successful evidence used to document the Measure for ESD is a copy of Form 315 that was sent to the TSP office.  If available provide evidence of receipt a TSP Authorization Code from the Office of Priority Telecommunications in a PPHR submission.
