Exercise Evaluation Procedures Example


This eight page PDF is an example of the exercise evaluation procedures used by the Eastern Idaho Public Health District in 2005 as evidence for Goal 3 of Project Public Health Ready.

This document is based on the IS-139 FEMA Independent Study Program Exercise Design Course designed to introduce you to fundamentals of exercise design and to prepare you to design and conduct a small functional exercise.  Included are blank charts for evaluator, checklists, narrative summary, key event response forms, problem logs, and an exercise debrief log.
