Training Plan Example - South Central District Health


This 20 page PDF is an example Training Plan submitted to PPHR in 2005 based on the 2004 criteria requirements for Goal 2: Work force Competency. The first page of this example is the template that South Central District Health Department used and a description of the types of information that is contained in the individual learning opportunity matrixes which together make up the training plan. Page three of the PDF clearly identifies the training plan priorities that were established by the LHD.
