Identifying Special Populations


PPHR Criteria: G1-M1-SMJ

This 43-page template for identifying and meeting the needs of special populations in communities was created in 2004 by the  Southwest District Health, ID for Goal 1, Measure 1 of Project Public Health Ready (PPHR).

This template includes definitions of various special needs groups and specific survey questions for each group that identifies:

  • The agencies that work with populations with special needs,
  • Preferred communication methods,
  • Potential barriers to communicating effectively, and
  • Unique preparedness planning issues.
