Task 12: Local PACE EH Action and Implementation Plan
This tool will be made available by making a request to the submitting agency.
Informed by results of PACE EH tasks 1-11, the action and implementation cycle includes the following actions:
Development of goals and objectives
Identification of contribution factors
Identification of possible interventions and prevention activities
Identification of community assets
Identification of potential barriers
Selection of interventions and activities
Determination of resource needs
Identification of potential partners
Provide training when appropriate
Development of timeline
Determination of measure s of success (performance measures)
Implementation of action plan
For each identified priority environmental health issue
