Example Epidemiology Annex


This Epidemiology Communicable Disease Annex was identified as an exemplary practice by Project Public Health Ready (PPHR) reviewers.  It includes sections on outbreak confirmation, assimilation of staff and volunteers, surveillance and reporting, analyzing and managing data, epidemiologic investigation tasks, and strike team roles and responsibilities. 

This document may be used as an example by PPHR applicants and LHDs interested in improving their level of preparedness.
