CDC's "Promoting Health Equity: A Resource to Help Communities Address the Social Determinants of Health" is a "workbook for public health practitioners and partners interested in addressing social determinants of health in order to promote health and achieve equality."

The workbook can serve as a guide for local jurisdictions to support the development of initiatives that address social determinants of health inequities, which is an essential component of health in all policies.

The usefulness of this document is in the abundance of examples of community driven and engaged projects addressing the social determinants of health. It is less helpful in providing guidance on HiAP.
