This guide, created by the Connecticut Department of Public Health, provides a framework for the development of a community health improvement plan (CHIP) and is easily adaptable for use by governmental community organizations.  The document explains in detail the steps in a planning process, the elements of a CHIP, and steps in priority setting and provides templates that can be tailored to individual agencies.  This is an excellent resource for those beginning to prepare for national accreditation and looking to develop a CHIP.
