Prerequisite: Healthy Carolinians Community Health Assessment Guide Book


This guidebook, created by the North Carolina Division of Public Health, Office of Healthy Carolinians/Health Education & State Center for Health Statistics, serves as an excellent resource for those beginning to prepare for national accreditation and starting the community health assessment (CHA) process.  This guide establishes a framework for conducting a comprehensive and collaborative assessment through nine easy-to-follow phases and provides additional tools and templates to help facilitate the process.
