Faith-Based Partnership Linkage Agreement


The Faith-Based Partnership Linkage Agreement, developed by the New York City Department of Health and Mental Hygiene, Bureau of STD Control, is an adaptable template that can be used to formalize agreements between local health departments and faith-based organizations. This template includes examples of how the local health department and faith-based organization will work together to address HIV and STD infection among the faith community, and can be tailored to the needs of a specific jurisdiction.
