Teaming Up to Confront Chronic Disease: Three National Approaches

Policy and System Changes - Impacts and Opportunities

This session will familiarize participants with population-based approaches that address chronic disease, specifically policy, systems, and environmental (PSE) strategies. Three national initiatives that use a collaborative approach to spur policy change include Action Communities for Health, Innovation, and Environmental changE (ACHIEVE), Pioneering Healthier Communities (part of CDC’s Healthy Communities Program), and the Y-USA Statewide Pioneering Healthier Communities Initiative funded by the Robert Wood Johnson Foundation. Participants will 1) learn why PSE approaches are more effective than traditional programs; 2) hear community examples of successful PSE strategies implementation; and 3) learn the capacities needed to begin working toward PSE change in their own communities.
